Liberty Private Care Services offers case management services for all patients. Case management services are available for any length of time, depending on need. Liberty’s case managers can assist you or your family with services including:

• Coordinating or arranging services
• Assisting with health promotion and education
• Developing a specialized care plan
• Assisting in times of crisis
• Developing lifestyle and social care programs
• Creating awareness of community support systems and assistance in contacting them
• Counseling and support
• Advocating on your behalf

Case management services are available for any adult who could benefit from professional oversight and care, and is paid by the user. Patients are provided telephone support for case management 24 hours a day, seven days a week. To begin case management services, click here.
