One of the key benefits gained through Liberty Hospice Services revolves around our continuity of care across North Carolina,South Carolina, and Virginia. To evaluate whether a patient should transition to hospice care, we consider a number of factors to determine the appropriateness of our service. This includes whether we believe:

•  An individual’s medical, nursing and social needs can be met adequately by our hospice team
• The patient’s residence supports his or her well-being

Liberty can make a personal evaluation of you or your loved one’s needs, and coordinate care options with your physician. Examples of candidates who meet our hospice criteria include:

• New physician diagnosis indicating hospice needs
• Recent exacerbation of a previous diagnosis or illness
• Final stage of a progressive disease
